Lakeland Health Care Center is Walworth County's 120 bed Skilled Nursing and Rehabilitation Sub-Acute Center. We provide care to medically complex residents with our team of terrific clinical team managers.
Our memory care household provides specialized care with an emphasis on individualized needs of our residents to make our facility their home.

Unit: Memory
Days: Sunday, Monday, Wednesday, Thursday, Friday (week 1)
Monday, Tuesday, Wednesday, Friday, Saturday (week 2)
including every other holiday
Work Hours: 10:30pm - 7:00am
Hours Per Shift: 8
Shifts Per Pay Period: 10
Position FTE: 1.0 FTE (Full-Time)

Essential Duties and Responsibilities:
Assess and evaluate the health status of residents and provide care and treatment in accordance with physician orders and standards of practice. Provide supervision, training, work planning/assigning and monitoring of nursing assistants to ensure that proper care is given.

Education and/or Experience
Associate's degree in nursing with zero to six months of relevant prior experience. Experience in restorative nursing, long-term care, psychiatric nursing, medical nursing or geriatric nursing preferred. Proven experience in ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry as well as ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Proven experience with medical terminology.

Certificates, Licenses, Registrations
Wisconsin Registered Nurse license through WI Department of Safety and Professional Services.
Criminal background check free of convictions of abuse of individuals or misappropriation of property while in a formal caretaking setting.
All staff of Lakeland Health Care Center may be called upon to respond to emergency situations assisting residents in their physical positioning.
If you are not available to work on the date this position starts, you will not be awarded the position.
All employees may be required to work overtime as determined by the department head.

Ability to frequently lift and move up to 75 pounds with aid of provided equipment, or a second person assist.
Ability to frequently lift, push or pull 50 pounds unassisted with bending and twisting of trunk.
Ability to communicate to residents using talking and hearing.
Ability to identify resident care needs through hearing and seeing.
Must be free from communicable disease.
"Constant" means at least 2/3's of the shift.
"Frequent" means at least 1/3 of the shift.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
